Self-refer your pregnancy to Mid Yorks Maternity

If you have changed your name or address and not informed your GP then please do not make a referral using this method until the GP has updated their system, please note you will also need your NHS Number.

Mid Yorks Maternity is an NHS service providing a single point of access for all maternity referrals in the Wakefield, Dewsbury and Pontefract area. The aim is to make sure you have access to the right care from day one. Mid Yorks Maternity will triage your referral based on the information you provide and the hospital will contact you to start your antenatal care. In the meantime if you are worried about your pregnancy, please tell your GP straight away.

Congratulations on your pregnancy! If you live in the Wakefield, Dewsbury, or Pontefract areas - or if you live out of area and wish to have your baby at The Mid Yorks Trust - then please use this self-referral form to book for our maternity services. If you live out of area, you will also need to contact your local midwife at your GP surgery. It is important that you contact us as soon as you know you are pregnant to ensure you get the best possible health support and are able to access free prescriptions, dentistry and other support so your care is personalised to your individual needs.
